Cosmetic Safety & Adverse Event Contact

This page explains basic product-safety precautions and how to report a suspected reaction, injury, defect, or other cosmetic product concern to USOLAB USA.
It is not a substitute for emergency or medical care.
 

1. Emergency Assistance

If someone collapses, has a seizure, has trouble breathing, or cannot be awakened, call 911 immediately. For a possible poisoning or unsafe exposure in the United States, contact Poison Control at 1-800-222-1222 or use webPOISONCONTROL at poison.org. Do not wait for an email response from USOLAB USA in an emergency.
 

2. General Safety Guidance

  • Use products only as directed and for their intended external cosmetic purpose.

  • Read the ingredient list, warnings, and directions before use. Do not use a product if you know you are allergic or sensitive to an ingredient.

  • Consider a patch test before first use, especially if you have sensitive skin or a history of reactions. A patch test does not guarantee that a reaction will not occur.

  • Avoid direct contact with eyes and mucous membranes unless the label specifically permits it. If contact occurs, rinse thoroughly and obtain appropriate medical advice.

  • Stop use if redness, swelling, burning, persistent irritation, rash, or another unexpected symptom occurs.

  • Keep products out of reach of children and store them according to package directions. Do not use products with damaged seals, unusual odor, contamination, or altered appearance.

  • If you are pregnant, nursing, under medical care, or using prescription skin treatments, consult a qualified healthcare professional before introducing a new cosmetic product.
     

3. How to Report a Product Concern

Email usolabusa@gmail.com with the subject line "Cosmetic Safety Report."

You may also write to the address below. Include as much of the following as possible:

  • Your name, email address, telephone number, and preferred contact method.

  • Product name, size, SKU, lot or batch code, expiration or manufacture information, and photographs of the label and packaging.

  • Order number, purchase date, place of purchase, and date first used.

  • Date and time the event began, how the product was used, amount and frequency, and any other products used at the same time.

  • A description of symptoms, body area affected, duration, outcome, and whether use was discontinued.

  • Any medical treatment, emergency care, hospitalization, diagnosis, or professional advice received.

  • Clear photographs of the product and affected area, if you are comfortable providing them.

4. What Happens After a Report

We may contact you for additional information, request that you retain or return the product and packaging, review manufacturing or distribution records, communicate
with the manufacturer or relevant business partners, and take other appropriate safety or quality actions. Submitting a report does not guarantee a refund and is not an admission of product defect or legal liability.
 

5. Regulatory Reporting and Records

When USOLAB USA is the responsible person or otherwise has a reporting obligation under applicable law, we will evaluate serious adverse-event information and submit required reports to the U.S. Food and Drug Administration within the applicable timeframe. Relevant records may be retained and disclosed as required for safety investigations, regulatory compliance, inspections, recalls, or legal obligations.
 

6. Product Recalls and Safety Notices

If a recall, market withdrawal, or important safety notice affects a product you purchased,
we may use available order and contact information to notify you and provide instructions. Follow recall instructions promptly and do not continue using an affected product.
 

7. Privacy

Health and safety information submitted with a report may be sensitive. We use and disclose it for safety review, customer support, regulatory reporting, legal compliance, and related purposes as described in our Privacy Policy.
